VACCINE INFORMATION
Franklin Primary Health Center is currently administering the COVID-19 vaccine to all individuals who are5 years of age and older. Anyone who is age 5 through 17 will be directed to locations that administer the appropriate Pfizer vaccine which is approved for pediatric use. Individuals can contact the Franklin Primary Health Center COVID-19 Vaccine Call Center to make an appointment or use the below online form to submit an application. We anticipate large call volumes and appreciate your patience when calling.
You can also download the below COVID-19 Vaccine Registration Form and email it to covidvac@franklinprimary.org. Please also attach a clear copy of your state ID and insurance card with your completed COVID-19 Vaccine Registration Form. Upon receiving your email with all the required information Franklin Primary Health Center will call you back with your appointment confirmation.

If you have been exposed to someone that tested positive for the Coronavirus (COVID-19) or if you are experiencing any of the following symptoms, please call our office prior to coming for your appointment or walking-in:
- Fever
- Cough
- Shortness of Breath
TELEHEALTH APPOINTMENTS
In an effort to provide our patients care during local quarantine, self-isolation and general social distancing measures, we now offer Telehealth appointments.
Telehealth gives us the ability to provide quality healthcare services safely and remotely to patients via telephone or computer.

TESTING
you have been exposed to someone that tested positive for the Coronavirus (COVID-19) or traveled to a high risk area and are exhibiting symptoms such as fever, cough or shortness of breath, you may be eligible for testing. Please call our Coronavirus Evaluation Hotline at 251-444-1122 for a phone evaluation.
Please note that all patients must complete a phone screening prior to testing. If you meet the criteria for testing, our staff will collect additional information over the phone and you will be assigned an appointment time for your test.
All approved testing will be conducted via drive-thru at the following centers.

Franklin Medical and Dental Express, City of Prichard, 216 E Prichard Ave in Prichard, AL.
Loxley Family Medical Center, 1083 East Relham Dr, Loxley, AL
Evergreen Family Medical Center, 316 S. Main Street in Evergreen, AL (Only on Thursdays)
Remember, you must have been pre-screened and assigned an appointment time in order to be tested. All others will be directed out of line and instructed to call the Evaluation Hotline at 251-444-1122.
