08/16/2024CALL CENTER REFERRAL SPECIALIST OpenParticipates in coordination, planning, implementation and evaluation of care provided by their assigned patient care teams by ensuring that referrals generated by the care teams are ordered, scheduled, completed or initiate appropriate patient follow up if appointment not kept. The Referral Specialist ensures that documentation is received by the care team from the specialty or referred provider; tracks all ordered tests and referrals until the referral cycle is completed for their assigned care teams; effectively communicates with assigned care team both verbally and by appropriate documentation in the patient’s record.
