VACCINE INFORMATION 

Franklin Primary Health Center will begin administering the COVID-19 vaccine to all individuals who are 18 years or older by appointment only. Anyone who is age 16 and 17 will be directed to locations that administer Pfizer vaccine which is approved for ages 16 and over. Individuals can contact the Franklin Primary Health Center COVID-19 Vaccine Call Center to make an appointment or use the below online form to submit the application. We anticipate large call volumes and appreciate your patience when calling.

You can also download the below COVID-19 Vaccine Registration Form and email it to covidvac@franklinprimary.org. Please also attach a clear copy of your state ID and insurance card with your completed COVID-19 Vaccine Registration Form. Upon receiving your email with all the required information Franklin Primary Health Center will call you back with your appointment confirmation.
