Social Worker (Wise Woman)

Location: Franklin Primary Health Center, Inc. – Mobile, AL

Franklin Primary Health Center is seeking a compassionate and dedicated Social Worker (Wise Woman) to join our team. This position provides comprehensive case management, psychosocial assessments, patient education, and care coordination as part of a multidisciplinary healthcare team. Essential Duties and Responsibilities

  • Conduct psychosocial assessments and identify patient service needs.
  • Develop individualized, patient-centered care plans.
  • Provide ongoing case management and monitor patient progress.
  • Coordinate referrals to community resources including financial assistance, housing, transportation, medical care, dental, vision, legal, vocational, and educational services.
  • Provide crisis intervention and supportive counseling as appropriate.
  • Maintain accurate and timely documentation in accordance with organizational standards.
  • Collaborate with physicians, nurses, behavioral health providers, and community agencies to coordinate care.
  • Recruit eligible patients for the WISEWOMAN and ABCCEDP programs.
  • Conduct WISEWOMAN support groups and assist with grant-related activities.
  • Complete required program documentation, reports, and data collection.
  • Participate in community outreach, health education, and departmental initiatives.
  • Perform other duties as assigned.

Qualifications

  • Degree from an accredited School of Social Work required.
  • Current Alabama Social Work license required.
  • Minimum of two (2) years of social work or case management experience preferred.
  • Valid Alabama driver’s license required.

  • Knowledge of social work practices, case management, and community resources.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to establish effective relationships with patients, providers, and community partners.
