Franklin Primary Health Center is seeking a compassionate and dedicated Social Worker (Wise Woman) to join our team. This position provides comprehensive case management, psychosocial assessments, patient education, and care coordination as part of a multidisciplinary healthcare team. Essential Duties and Responsibilities
- Conduct psychosocial assessments and identify patient service needs.
- Develop individualized, patient-centered care plans.
- Provide ongoing case management and monitor patient progress.
- Coordinate referrals to community resources including financial assistance, housing, transportation, medical care, dental, vision, legal, vocational, and educational services.
- Provide crisis intervention and supportive counseling as appropriate.
- Maintain accurate and timely documentation in accordance with organizational standards.
- Collaborate with physicians, nurses, behavioral health providers, and community agencies to coordinate care.
- Recruit eligible patients for the WISEWOMAN and ABCCEDP programs.
- Conduct WISEWOMAN support groups and assist with grant-related activities.
- Complete required program documentation, reports, and data collection.
- Participate in community outreach, health education, and departmental initiatives.
- Perform other duties as assigned.
Qualifications
- Degree from an accredited School of Social Work required.
- Current Alabama Social Work license required.
- Minimum of two (2) years of social work or case management experience preferred.
- Valid Alabama driver’s license required.

- Knowledge of social work practices, case management, and community resources.
- Strong interpersonal, organizational, and communication skills.
- Ability to establish effective relationships with patients, providers, and community partners.
