Position Summary

Franklin Primary Health Center is seeking a compassionate and dedicated Licensed Bachelor Social Worker (LBSW) to join our Behavioral Health team. The LBSW provides psychosocial assessments, case management, care coordination, patient education, and referrals to community resources. Working collaboratively with an interdisciplinary healthcare team, this position helps patients overcome barriers to care while improving health outcomes and access to community services.

Essential Duties and Responsibilities

  • Conduct psychosocial assessments to identify patient needs and barriers to care.
  • Provide comprehensive case management and care coordination services.
  • Coordinate referrals to community agencies and supportive resources.
  • Assist patients with housing, transportation, financial assistance, food resources, and other community services.
  • Collaborate with physicians, behavioral health providers, nurses, and other interdisciplinary team members.
  • Maintain accurate, timely, and complete documentation within the electronic medical record.
  • Participate in quality improvement initiatives and community outreach activities.
  • Maintain HIPAA compliance and protect patient confidentiality.
  • Perform other duties as assigned.

Qualifications

  • Bachelor of Social Work (BSW) from a CSWE-accredited program required.
  • Current Licensed Bachelor Social Worker (LBSW) license in the State of Alabama required.
  • One (1) year of healthcare, behavioral health, case management, or social services experience preferred.
