Licensed Practical Nurse (LPN)
Job Summary:
Participates in the coordination, planning, implementation, and evaluation of care provided by the Patient Care Teams under the direction of the provider team leader, and within the objectives, standards, and policies of the center, as well as within the parameters of his or her preparation.
Essential Functions:
- Participates in care team huddles and initiates pre-visit planning workflow to ensure care completion prior to the visit whenever possible.
- Exhibits excellent customer service at all times.
- Maintains a professional appearance and attitude when interacting with internal and external customers.
- Monitors and remains aware of patient wait times at all times.
- Performs patient assessments for non-appointment patients, including walk-ins and phone calls.
- Prioritizes walk-in patients according to the urgency of the patient’s condition.
- Assesses vital signs, including temperature, pulse, respiration, blood pressure, weight, and height.
- Places patients in examination rooms with proper draping and privacy.
- Prepares patients for examination procedures.
- Keeps exam rooms clean, organized, and properly stocked.
- Performs comprehensive assessments, including documentation of symptoms, chief complaints, medication reconciliation, and allergy history.
- Initiates standing orders in accordance with established guidelines adopted by FPHC.
- Administers medications as authorized and prescribed by the physician.
- Properly documents all treatments, medications, lab results, phone messages, referrals, and other patient-related information.
- Assists the physician with treatments, examinations, injections, x-rays, EKGs, holter monitors, and assessments.
- Assists with and responds to emergencies.
- Monitors supply levels and reorders when needed.
- Ensures coordination and tracking of all labs, referrals, and diagnostic tests as required.
- Maintains the crash cart, oxygen tanks, narcotic box, refrigerator, medical supply room, medication closet, and IV cart.
- Completes and reports findings on log sheets to the supervisor at the end of the reporting period.
- Engages patients in activities to improve their health through assessment, teaching, education, and self-management support.
- Participates in PCMH Care Team quality improvement activities, including care team performance on required clinical, operational, and financial measures.
- Performs venipuncture and basic lab work as needed.
- Performs other duties as assigned.
Pediatrics:
- Administers immunizations, PKUs, Medicaid screenings, vision and hearing exams, TB skin tests, and lab work when necessary.
- Performs strep tests and obtains urinalysis specimens.
OB/GYN:
- Assists with colposcopy and cryosurgery procedures.
- Assists with Norplant insertion, OB intake, family planning, TB skin tests, and lab work when necessary.
Additional Responsibilities:
- Maintains a current Alabama nursing license and CPR certification.
- Remains up to date on current employee health practices and guidelines through continuing education classes, journals, and professional organizations.
- Attends continuing education related to infection control practices, monitoring, and surveillance of infectious disease processes within the organization.
- Maintains a professional approach and strict confidentiality.
- Observes and supports center policy.
- Demonstrates promptness, efficiency, and minimal absences.
- Carries out additional duties as assigned.

Education and Experience:
Diploma in Nursing from an accredited school of nursing required. Current license to work in the State of Alabama required. One year of clinical work experience preferred.
