Full-time Overview:
Franklin Primary Health Center is seeking a Licensed Practical Nurse (LPN) to support patient care in Baldwin County, AL. This full-time role includes clinical coordination, direct patient care, and support for daily operations under the supervision of provider team leaders.
Key Responsibilities:
– Participate in care team huddles and initiate pre-visit planning
– Deliver excellent customer service and maintain a professional appearance
– Monitor patient wait times and provide timely triage and assessments
– Take and record vital signs including temperature, BP, weight, and height
– Prepare patients and exam rooms for procedures
– Maintain clean, organized, and stocked exam rooms
– Perform comprehensive assessments, including medication reconciliation and allergy documentation
– Initiate standing orders per FPHC guidelines
– Administer medications as prescribed
– Document treatments, phone messages, labs, and referrals accurately
– Assist with exams, injections, EKGs, and minor procedures
– Respond to emergencies and assist with crash cart readiness
– Monitor and restock supplies and maintain clinical equipment
– Coordinate lab, referral, and diagnostic test follow-ups
– Engage patients in care planning, education, and self-management
– Participate in PCMH quality improvement and performance tracking
– Perform venipuncture and basic lab procedures
– Carry out other duties as assigned
Additional Responsibilities:
– Maintain current Alabama LPN license and CPR certification
– Stay up-to-date on infection control and clinical best practices
– Observe and adhere to center policies
– Maintain professional behavior, confidentiality, and punctual attendance
Qualifications:
– Diploma from an accredited nursing program
– Current LPN license in Alabama
– CPR certification required
– One year of clinical experience preferred
– Strong communication, clinical, and organizational skills
– Valid driver’s license
Work Schedule & Pay:
Full-time | Monday–Friday | Day shift | $20.00–$25.00 per hour
