JOB SUMMARY:

Participates in coordination, planning, implementation, and evaluation of care provided by the Patient Care teams under the direction of the provider team leader and within the objectives, standards, and policies of the center. The LPN Team Lead assumes leadership in the absence of the Director of Nursing, Deputy Director of Nursing, or the RN.

ESSENTIAL FUNCTIONS:

  • Participates in care team huddles and initiates pre-visit planning workflows.
  • Maintains excellent customer service, professional appearance, and attitude at all times.
  • Monitors patient wait times and addresses delays.
  • Performs patient assessments for walk-ins and phone calls, prioritizing based on urgency.
  • Assesses vital signs including temperature, pulse, respiration, blood pressure, weight, and height.
  • Prepares patients for examinations and ensures proper privacy.
  • Keeps exam rooms clean, organized, and stocked.
  • Completes comprehensive patient assessments including documentation of symptoms, medications, and allergies.
  • Initiates standing orders per FPHC guidelines.
  • Administers medications as prescribed by the physician.
  • Accurately documents treatments, labs, referrals, and communications.
  • Assists physicians with procedures, exams, injections, EKGs, and other clinical duties.
  • Responds to emergencies as needed.
  • Monitors and maintains supply levels.
  • Tracks labs, referrals, diagnostics, and abnormal findings.
  • Maintains clinical equipment including crash cart, oxygen tanks, medication storage, and IV carts.
  • Promotes patient engagement through education and self-management support.
  • Participates in PCMH quality improvement initiatives.
  • Performs venipuncture and basic lab work.
  • Ensures adequate staffing coverage and supports nursing leadership.
  • Provides day-to-day supervision of nursing staff.
  • Prepares staff schedules.
  • Tracks and compiles department data for reporting.
  • Performs other duties as assigned.

PEDIATRICS:

  • Administers immunizations, PKUs, Medicaid screenings, and TB tests.
  • Performs vision, hearing, strep, and urine testing.
  • Orders immunizations through the VFC Program and submits required data.

OB/GYN:

  • Assists with colposcopy, cryosurgery, Norplant insertion, and OB intake.
  • Supports family planning services and related lab work.

ADDITIONAL RESPONSIBILITIES:

  • Maintains current Alabama Nursing License and CPR certification.
  • Participates in continuing education and infection control training.
  • Maintains confidentiality and professionalism.
  • Supports policy compliance and JCAHO readiness.
  • Maintains consistent attendance and punctuality.

PERFORMANCE STANDARDS:

Performance will be evaluated annually by the Director of Nursing or designee. A satisfactory rating is the minimum acceptable level of performance.

HEALTH AND SAFETY REQUIREMENTS:

  • Adheres to safety regulations and policies.
  • Completes annual safety and Hazard Communication training.
  • Maintains compliance with OSHA and CLIA standards.
  • Maintains required health records including TB testing and hepatitis series.

PRIVACY RULE COMPLIANCE REQUIREMENTS:

  • Adheres to HIPAA policies and procedures.
  • Completes annual privacy training.
  • Protects patient confidentiality and maintains secure records.

QUALIFICATIONS:

Education and Experience:
Diploma in Nursing from an accredited school required. Current Alabama LPN license required. IV certification or infusion experience required. One year of clinical experience preferred.
